Use this checklist to determine your eligibility to use this form:
- I have had a change in my name and my most
recent U.S. passport book and/or U.S. passport card was issued less
than one year ago;
- My identifying information in my most recent
U.S. passport book and/or U.S. passport card was printed incorrectly; or
- My most recent U.S. passport book was
limited in validity and was issued less than one year ago.
If it is MORE than one year since the date of issuance of your
passport card, you must submit Form DS-82
(or DS-11, if appropriate) and pay the
applicable fees. For more information on Form DS-82,

Send completed form DS-5504 with your MOST RECENT U.S. passport
book and/or passport card, any required additional evidence, and two
recent color photographs to:

Other Important Information Regarding the DS-5504:
- THERE IS NO FEE TO HAVE A U.S. PASSPORT
AMENDED. Your amended U.S. passport and any documentary evidence
submitted will be returned to you via priority mail. You will
receive your amended passport within 5-6 weeks.
- For faster processing, you may request
Expedited Service. Expedited requests will be processed within three
working days from receipt at the National Passport Processing Center. The fee
for this service is $60 per application and 2-way overnight
delivery. The expedite fee does not
include overnight delivery. Customer must arrange for
overnight delivery to ensure receipt within 3 weeks. Enclose
the $60 fee in the form of a personal check or money order. All fees
should be payable to the "U.S. DEPARTMENT OF STATE." Do not send
cash. This service is available only in the United States. Please
write "EXPEDITE" in lower left corner of the mailing envelope.
- If you desire overnight return delivery,
include appropriate postage fees or a pre-paid overnight return
envelope. NOTE: Passports will NOT be mailed to private addresses
outside of the United States.
