two (2) hours of continuing education in the areas of Competent
Person, Trenching, Excavation, Confined Space or Lock-Out Tag-Out or
Traffic Work Zone safety training for each on-site safety employee.

listed below. (2) Select DMA Forms & Materials on left side of
screen. Select Review Forms & Instructions. Select DMA Forms "DMA
for Funding & Business Contracts". Print form, complete and return
with application.
