Contractor Registration Requirements
The information below is the requirements for a Contractor's Registration:
- Complete 2010-2011 Application - effective July 1, 2010 thru June 30, 2011 (PDF, 27KB)
- Submit testing fee of $25.00 for each on-site safety employee.
- Submit Terrorist Exclusion Form: Instructions to get to form - (1) select "Terrorist Exclusion List (PDF, External Site)" on the left side of screen. Print and attach to the form listed below. (2) Select DMA Forms & Materials on left side of screen. Select Review Forms & Instructions. Select DMA Forms "DMA for Funding & Business Contracts (PDF, External Site)". Print form, complete and return with application.
- Submit Project Safety Agreement (PDF, 75KB)
- Schedule test appointment for each on-site safety employee.
